Notice Regarding 2025 Form 1095-B and 1095-C Availability

 

This notice is to inform you that for the 2025 tax year, you may request copies of your Form 1095-B and/or 1095-C, if applicable.

 

•   Form 1095-B: This form provides information about your health insurance coverage for the year.

•   Form 1095-C: This form provides information about the health insurance coverage offered to you by Pathways.

 

To request your form(s), please submit a written request to Kris Woolford at kwoolford@up-pathways.org  Please include the following information in your request:

 

•   Your full name

•   Your Social Security Number (SSN) or Employee ID

•   The tax year for which you are requesting the forms (i.e., 2025)

 

We will process your request within 30 days of receiving it.

Important Note: You are not required to request these forms if you do not need them for tax purposes. The IRS no longer requires individuals to submit their health insurance coverage information to them.
